Ontario Health Teams (OHTs) are a new model of health care delivery that puts patients, families and caregivers at the centre of the system.
Muskoka Almaguin Ontario Health Team is a collaboration of local healthcare providers and organizations working together to improve patient outcomes and population health, maximize the healthcare system and improve the clinical staff experience.

How We Help
Currently, Muskoka Almaguin OHT’s scope of services focuses on improving care coordination and care transitions for priority populations (currently those 65 and older). We plan to continue to expand the continuum of services through new partnerships and collaboration with long-term care and palliative care.
