BACKGROUND

The Muskoka and Area Ontario Health Team (MAOHT) is a collaboration of Muskoka and area health and social/community service partners and includes representation from 13 organizations, reflecting primary care, home care, hospitals, community agencies, mental health and addictions, the District Municipality of Muskoka and patient, family and caregiver partners.

POSITION DESCRIPTION

CCFHT is recruiting for a full time nurse practitioner on behalf of the Muskoka and Area Ontario Health Team. The position will work in collaboration with Muskoka Algonquin Health Care and other local and regional partners to support and expand the Heart Function Clinic. The candidate will help integrate COPD clinic into this model as the MAOHT builds and strengthen clincal care pathways for our community using a chronic disease management model. MAOHT has established a relationship with the Simcoe Muskoka Regional Heart Program Heart Function Clinic at the Royal Victoria Regional Health Centre, who will ensure mentorship and support to the successful candidate.  This position works with a team of allied health and community partners to provide support to patients with heart failure. MAOHT is willing to support candidates with a stong interest to delvelop the knowledge and skills required for this role.            

Including but not limited to:

PATIENT CARE

  • Works within the legislated scope of practice of the Nurse Practitioner, determines the need for, orders and interprets reports of appropriate laboratory and diagnostic imaging.
  • Collaborates with hospital internists and local primary care providers to enable comprehensive care and referral for congestive heart failure patients within Muskoka and Area.
  • Conducts patient assessments and develops care plans for all referred CHF patients triage as appropriate to other care settings.
  • Provides direct comprehensive care to patients with specialized and complex needs.
  • Ongoing management of HF patients (NYHA Class II and III)
  • Consultation and referral as appropriate to the RVH consultant and HF Clinic (Node)
  • Performs the responsibilities of the position consistent with the operational policies of the CCFHT.
  • Performs the responsibilities of the position consistent with the operational policies of the CCFHT.
  • Mangement of co-morbid conditions impacting HF, collaboration with palliative care.

CLINICAL LEADERSHIP

  • Ongoing development and optimization of clinic practices, policies and care pathways
  • Planning and coordinating care amongst multiple disciplines including nurses, community paramedics and support staff
  • Representation of clinic in associated committees or partnerships
  • Participate in OH Community of Practice in collaboration with the clinic’s Program Manager

QUALIFICATIONS

  • Current registration as a Registered Nurse in the Extended Class [RN(EC)] without limitations from the College of Nurses of Ontario. 
  • Current certification in Advanced Cardiac Life Support required
  • Additional education and certification in area of specialty preferred
  • Membership in the Registered Nurses Association of Ontario (RNAO) or Nurse Practitioner Association of Ontario (NPAO) organizations
  • Preference for candidates with experience in community or ambulatory care heart failure clinic development and management.
  • Experience in or strong foundation in chronic disease management that includes heart failure and related co-morbidities.
  • Experience working in an advanced clinical practice role required, preferably as a Nurse Practitioner or a Clinical Nurse Specialist
  • Experience in nursing research methodology preferred
  • Valid driver’s license
