Come and fulfill your dream of becoming a comprehensive, primary care nurse practitioner in beautiful Dorset, Ontario.  We are looking for a 12-18 month contract providing primary care in an autonomous fashion at the top of your scope, with the support of a comprehensive team of providers. Live, work and develop, this role can be tailored to your unique needs and practice goals. 

The Nurse Practitioner provides primary health care with a strong emphasis on health promotion and disease prevention within the scope of practice and standards of care outlined in The Practice Standard – Registered Nurses in the Extended Class, as per the College of Nurses. The NP is responsible and accountable for the management of their practice and is legally bound by the Standards of Practice outlined by the College of Nurses of Ontario.

AFHT Primary Care Nurse Practitioners have their own roster of patients. The full-scope Primary Care Nurse Practitioner makes clinical assessments, diagnoses, and manages physical and mental health challenges and disease processes for individuals of all ages. AFHT Primary Care Nurse Practitioners work in the community, both independently and in collaboration with the AFHT and health care partners as appropriate.

Organizational structure:

Reports directly to Executive Director, AFHT.

Liaison with Lead Physician, AFHT.

Partners and collaborates with Physicians and interdisciplinary health care providers of the AFHT and other health care agencies.

 Position Requirements:

Certificate of competence from the College of Nurses of Ontario in the Extended Class.

Demonstrated evidence of continuing education with emphasis on advanced clinical nursing practice skills related to primary health care, preferably in a rural and northern setting.

Ability to problem solve independently, and to consult when appropriate. 

Able to facilitate positive relationships.

Demonstrated exceptional attendance levels, work performance, disciplinary history and positive work references.

Excellent oral and written communication, organization, leadership, and interpersonal skills.

Demonstrate a willingness to learn new skills as they pertain to the community being served.

Maintains the confidentiality and privacy of all patient related and FHT information.

Proficiency with computers and associated software, Telus PSS a strong asset.

Additional formal managerial training would be an asset.

Impeccable communication skills and a willingness to work collaboratively.
