SCOPE (Seamless Care Optimizing the Patient Experience) is a virtual, interdisciplinary program connecting primary care providers (PCPs) in the community to real-time supports when you need it most. Through a single point of access, the MAOHT’s SCOPE Nurse Navigator and Mental Health Social Worker are available to provide PCPs system navigation specific to the healthcare system and mental health supports. New Heart Care Support for Your Practice
The Muskoka Almaguin Ontario Health Team (MAOHT) is pleased to introduce the Best Care Program—a new initiative to streamline heart failure management in primary care settings.
What is the Best Care Program?
The Best Care Program is designed to support primary care providers and their patients with congestive heart failure. By embedding a specialized Case Manager in your office, this program provides personalized assistance with heart failure diagnosis, treatment optimization, medication management, and patient education, all delivered close to home.
Benefits to Your Practice
- Enhanced Patient Outcomes: Patients receive comprehensive, ongoing care that reduces the need for hospitalization and escalated care.
- Streamlined Coordination: Case Managers ensure continuity of care, collaborating closely with your practice and cardiac nurse practitioners.
- Resource Support: Access to Certified Heart Failure Educators and a coordinated pathway means more time for other patients and optimized care delivery.
The Heart Function Clinic, an initiative of the Muskoka Almaguin Ontario Health Team, provides specialized cardiac care in collaboration with Best Care and Muskoka Algonquin Healthcare (MAHC). Located at the South Muskoka Memorial Hospital (SMMH) site, the clinic’s Advanced Cardiac Nurse Practitioner offers personalized support to patients with heart conditions, with an escalation pathway to a cardiologist in the Barrie area if needed. This team-based approach ensures patients receive consistent, expert care to manage their heart health effectively.
Referrals can be submitted through the Ocean e-referral platform or by using the referral form on the MAHC website.
This resource guide provides healthcare providers with information and guidance for navigating transgender and non-binary healthcare and support services. All services included are available to those residing in Muskoka and the surrounding area (unless stated otherwise).
