We are pleased to announce that Sarah Hudson has been selected to take on the role of Palliative Care Clinical Coach with the MAOHT effective June 23, 2025.
Sarah brings a wealth of experience to this role, having dedicated her career to palliative care across Muskoka and Almaguin—first as a PSW, then as an RPN, working with Family Health Teams, Home and Community Care (now Ontario Health atHome), and the North Simcoe Muskoka Hospice Palliative Care Network.
Over the past five years at Hospice Muskoka, Sarah has served as Clinical Lead at Andy’s House and more recently as Community Programs Director. She has been instrumental in leading accreditation efforts and implementing the Meditech Expanse EMR in collaboration with RVH and local hospice partners.
As a Clinical Coach, Sarah will work with partners across the health system to build an equitable, accessible, and timely palliative care referral process for everyone in our region. Her responsibilities will include:
• Leading process improvements with hospitals, primary care, community paramedics/EMS, and Ontario Health atHome.
• Enhancing palliative care delivery by educating and supporting teams in Long-Term Care, Retirement Homes, and community care.
• Developing early identification and referral pathways in partnership with primary care.
• Establishing a robust data collection process to better capture the palliative care services provided across all partner organizations.
Please join us in welcoming Sarah to this new role.
